How it works
We keep care coordination honest and simple. Your doctor’s office sends the orders, and we handle the rest.
1. Your provider orders care
Your healthcare provider faxes us the orders. We also accept Rightfax (e-fax). Once they arrive, we start your care plan.
2. Your first visit
A nurse or therapist calls to schedule a full evaluation, within 24 to 48 hours of receiving orders from your doctor or your discharge from the hospital.
3. Your care plan
Your provider designs your plan and sets how often and how long we visit. We coordinate the details and send them a summary after every visit.
Less paperwork for your office
Our team has worked in many hospital roles, so we know the paperwork doctors face.
We take that load off your office and keep your patients’ care coordinated.
