Our Services
Our expert team provides a range of personalized home health services in Miami-Dade and Monroe counties, including skilled nursing, rehabilitative therapies and assistance with daily activities. We also offer specialized cancer and supportive care treatments.
How it works
We believe in honest, transparent, no hassle care coordination. Your doctor’s office will provide us with the orders and we’ll take care of the rest.
Your healthcare provider can order home health care by submitting the appropriate documentation via fax. Our office also supports the use of Rightfax (i.e. e-fax). Once we receive your orders, we’ll start designing your care plan.
A nurse or therapist will call to schedule your first visit, where they’ll conduct a thorough evaluation to determine your best course of action. We’ll arrange for this to happen within 24 hours of receiving orders from your doctor’s clinic or being discharged from the hospital.
The services we provide will be based on a personalized plan developed by your healthcare provider. Your doctor will determine the frequency and length of each visit and we’ll coordinate all the details. After each visit, we’ll report back to them with a summary of care.
