Our team will work closely with you, doctors and caregivers, whether it be family members or friends, to treat or help you recover from illness, injury or surgery, always following the personalized care plan developed by your healthcare provider

Home health care is also an ideal treatment method for those suffering from chronic illness, at-risk complications from apre-existing condition or who simply need assistance around the house. Home health care can be ordered through your healthcare provider, who will indicate whether you need skilled nursing care, a home health aide or physical, occupational, and/or speech therapy. Your eligibility will be determined based on your condition and ability to function at home independently.

During your first visit, a nurse or therapist will complete a thorough evaluation. This evaluation will assist us in identifying where you can benefit from education and tools to manage your health. We’ll then work with your doctor to determine which services will best suit your needs, as well as how long and how often visits should take place.

Visit frequency is based on your personalized care plan. Your doctor may adjust your care plan as needed based on your overall progress and recovery.

Yes! Federal law gives patients the right to choose their health care providers. If you are interested in working with Oasis Health Partners, let your doctor know and they can begin the referral process.

After receiving your doctor’s order and connecting with their office, a nurse or therapist will contact you by phone to schedule your first visit within 48 hours of being discharged from the hospital.

Medicare covers 100% of the cost of services for eligible patients. Your state’s Medicaid program or private insurer may also cover some if not all of the costs. We recommend contacting your private insurer for details as each plan differs.

All staff members require a background screening and must provide proof of licenses and certifications applicable to their role. Employees go through an onboardingprogram specifically designed for Oasis Health Partners that aligns with our mission and vision.

Hospitalization is NOT a requirement. Patients are referred by their doctors for a number of reasons, from managing chronic conditions to needing physical, occupational or speech therapy.

Just because your insurance doesn’t cover a specific service or treatment doesn’t mean you can’t benefit from it.
