A quality life begins with quality care
At Oasis Health Partners, we love helping people. From the caregivers who lean on us to the doctors and case workers who depend on our services, we are here to support our patients and the entire community that revolves around their care.
Patients heal better at home when they sleep in their own bed after a home cooked meal and when they’re surrounded by the people and things they love most. But finding the right home health agency can be challenging for a number of reasons: large amounts of paperwork delay visits, nurses and aids may lack the training needed to attend to specific conditions, and doctors and case workers are often tasked with coordinating multiple agencies to deliver the appropriate treatment.
These challenges risk many patients being readmitted to the hospital. We are a home health agency built by nurses and healthcare leaders who consistently witnessed these challenges. We have also felt the pressures of being family caregivers ourselves. We are here to facilitate the transition from the hospital or clinic to home by collaborating with our community partners and ensuring it’s a smooth process.
A quality life begins with quality care
Our Mission
Our mission is to alleviate the impact of illness through compassionate and highly skilled care, provided efficiently and hassle free within the comfort of home. We do this by fostering a trustworthy team of trained medical professionals who are fully committed to seeing our patients through their treatment and recovery. Think of us as an extension of your doctor’s office; they design your treatment plan and we handle the rest, from filing paperwork to coordinating visits–and everything in between.
Our Values
Our aim is to consistently deliver top-notch care.
We are committed to providing truthful, transparent and evidence-based care.
Every interaction we have reflects our genuine enthusiasm for seeing patients through their treatment and recovery.
We believe in fostering a trusting, harmonious partnership between our teams, patients and the doctors and healthcare professionals who depend on us.
Healthcare can be confusing. We focus on simplifying the process so doctors and patients can focus on getting better.
A home health agency led by healthcare experts
Maritza Alencar
“There’s no place like home to heal. As a medical professional, mother of two and caregiver to a parent with chronic needs, I understand how critical it is for patients and caregivers to receive the specialized care they deserve without delays, setbacks and preventable hospital stays.”
My team fully understands the complexities and gaps in the system, so we set out to create a company that excels at delivering compassionate, personalized care, acting as a true ally and facilitator to doctors, healthcare providers and caregivers.
